Amikacin powder chemical called ο- 3-Amino-3-deoxygenation- α- D-glucopyranosyl - (1 → 6)- ο-〔 6-amino-6-deoxy-D-glucopyranosyl - (1 → 4)] - N '- (4-amino-2-hydroxy-1-oxybutyl) - 2-deoxy-D-streptomycin. The potency per 1mg of dried product shall not be less than 910 amikacin units, melting point is 203-204ºC (sesquihydrate).  Almost odorless and tasteless. Its mechanism is to act on ribosomes in bacteria, inhibit bacterial protein synthesis, and destroy the integrity of bacterial cell wall, resulting in bacterial cell membrane damage and cell death. It is applicable to infections caused by gram-negative bacilli and penicillin-resistant Staphylococcus aureus.

Function

The mechanism of action of amikacin is to act on ribosomes in bacteria, inhibit bacterial protein synthesis, and destroy the integrity of bacterial cell wall, resulting in bacterial cell membrane damage and cell death. It is applicable to infections caused by gram-negative bacilli and penicillin-resistant Staphylococcus aureus. For intramuscular or intravenous infusion, the half-life of intravenous infusion is about 2h, and it can be as long as 30h in the absence of urine, and it rarely combines with protein. It mainly causes cochlear nerve damage, and the ototoxicity and nephrotoxicity are similar to gentamicin. Amikacin plays an antibacterial role by blocking bacterial protein synthesis by binding to bacterial 30S subunit.

Application

It is mainly used for various infections caused by gram-negative bacteria resistant to gentamicin and kanamycin, such as Escherichia coli, Proteus and Pseudomonas aeruginosa.

 

The common adverse reactions of Amikacin were:

1. It mainly affects the cochlear nerve, causing the patient's high-frequency hearing to be damaged first, and then the hearing loss gradually develops to deafness, tinnitus and the feeling of fullness in the ear.

2. Nephrotoxicity: It mainly damages the proximal renal tubules, which may lead to proteinuria and tubular urine, followed by hematuria, decreased or increased urine volume, and then to azotemia, decreased renal function, increased potassium excretion, etc.

3. Neuromuscular block: amikacin has the effect of blocking acetylcholine and complexing calcium ions similar to curcumin, and can cause myocardial inhibition, respiratory failure, etc. Patients with original myasthenia or who have received muscle relaxants are more likely to occur.

4. Allergic reactions: A few patients may have allergic reactions after medication, including anaphylactic shock, rash, urticaria, drug fever, etc.

5. Others: Amikacin can interfere with normal flora, and long-term use can cause excessive growth of non-sensitive bacteria.
